Reports reveal lax adherence to and enforcement of safety procedures in the months leading up to the incident.
Biological defense mishap reports filed at USAMRIID between April 1, 2002 and Dec. 1, 2005 reveal continued safety problems, suggesting the institute needs additional improvements in safety monitoring and enforcement.
USAMRIID has made many changes to its biological safety and security program to prevent a similar incident from happening again.
